New Patients & How to Register

We welcome new patients who live in our catchment area
 

There are 2 ways to register - either online OR in person. 

(Please note that if you are registering online you will still need to bring your registration forms and photo ID into the surgery to complete your registration.)

Please remember:

  • To bring along a form of PHOTO ID with you e.g. Driving Licence, Passport.
     
  • Please bring the full details of your previous GP and previous address.
     
  • It can take up to 6 weeks to receive your medical notes from your previous practice.
     
  • Please register your new baby immediately. (When registering your new baby please bring a copy of their NHS number and state the ethnicity – we need this to register effectively)
     
  • Please notify us if you change your name, address, or telephone number.

After registering all new patients are offered an appointment for a new-patient check with our Healthcare Assistant. This enables us to gather some information about you and any medications you may require before we receive your notes. You will be required to bring a urine sample in for your new patient check. If you are currently taking medications, please bring your current medication details or right hand side of you prescription to an appointment with a GP.

For your continued care it is very important that you let us know if there are any changes to your address or telephone number. If you move outside of our practice area you will be asked to register with another GP closer to your new address.
