New Patients are always welcome at St. Luke's

Before joining the practice we ask that all patients fill in a confidential health questionnaire.

Please print it off, fill in and bring to the surgery. It’s also important to bring along a form of ID with you e.g. Driving Licence, Passport.

The receptions will ask you to fill in a NHS Registration form. A new patient check will be arranged with one of our Nurses.
